If the client`s therapist hires at Eat Speak Learn, the provider will do everything in their power to continue providing services with another therapist, but may have to suspend or terminate the service contract if no other therapist can be found. If multidisciplinary team interventions occur, all service providers will individually assert claims against a single support item. If a customer wishes to provide feedback to Eat Speak Learn or is not satisfied with the provision of services and wishes to file a complaint, the customer should contact Eat, Speak Learn by phone at 02 6156 2804, by email firstname.lastname@example.org or in person at Northpoint Plaza, 8 Chandler Street, Belconnen, ACT, 2617. Service contracts are established after an initial consultation at the Eat Speak Learn Clinic and generally include an assessment, therapeutic report and intervention meetings in accordance with the fee schedule, unless otherwise agreed between the two parties. A maximum of 30 minutes of additional intervention time and/or follow-up services may be charged per intervention meeting under this Agreement. Follow-up services are broken down and billed according to the service in addition to the intervention meetings. All voice pathologists working with NDIS-funded clients must have a signed service contract with each client. If changes are necessary to the Services or their provision, the Parties agree to discuss and review this Service Agreement. The parties agree that all changes to this Service Agreement will be in writing, signed and dated by the parties. Therapeutic offers are intended to adapt, adapt and strengthen the Community`s capacity for participation.
Eat Speak Learn offers tailor-made voice pathology services and recommends best practice therapeutic help to achieve the client`s goals. The client may also contact the parent/guardian/guardian or plan manager or any other party with the right to manage services on behalf of the client. The signatory of this agreement must be over 18 years of age and will be the customer or representative of the customer. The signatory is responsible for the customer`s contractual obligations under this service agreement. It is the customer`s responsibility to inform the provider if the provision of services is not satisfactory immediately after the provision of services or within a period agreed in writing between the two parties. The lack of information in response to dissatisfaction means that services have been provided satisfactorily. If a customer does not make a short-term cancellation after 24 hours prior to service or if they do not attend an appointment, Eat Speak Learn will charge a cancellation fee. The cancellation fee is 90% of the scheduled appointment time and 100% of the travel time. Cancellation fees are waived if there must be a medical certificate attesting that the client or guardian has not been able to participate in the scheduled service..
